SELF, a new and fun take on the health care experience geared towards those experiencing diabetes and are looking for information, assistance and so much more. This projects includes a Planner, Diabetes Glossary, Branded Informational Flyers, Poster, Gift Basket, Branded Binder, Table Cloth, Application, and Trivia Game. SELF stands for Social, Educational, Local, Fun and was created by myself and two other Designers. My contribution to self are the publication pieces:
A binder which contains custom dividers for storing flyers and information gathered from the fair. This binder is meant to store more than information, it is to act as a container for a patient's own personalized care plan and help them better care for themselves.
A glossary dedicated to all the terminology and vocabulary that may be encountered during one's journey with diabetes. This piece helps break down the language into smaller pieces for those without medical degrees to be able to understand.
A planner for doctors appointments, community events, exercise plans and more. This will help the patient create and stick to their own personalized care plan that fits them.
Lastly, mock flyers and informational articles to help patients stay informed about their diagnosis and continue to make informed decisions for themselves, as well as stay up to date about new care methods and technology.
