The original "Personal Property Loan Agreement" form from Portland State University was a for that lacked clarity on how to fill out the form. Through a redesign, I was able to create an easier to use form. 

It is more clear on how you should be filling out the form as well as accessibility friendly with text form fields and screen reader friendly.
